What is orgasm difficulty?
Female orgasmic disorders are characterized by a recurrent delay in orgasms, decreased frequency of orgasms, absence of orgasms, or a markedly reduced intensity of orgasms, despite adequate sexual stimulation.

Types of female orgasm disorders
Primary — Has never experienced orgasm under any circumstances.
Secondary — Previously experienced orgasm but currently has difficulty achieving it.
Generalized — Occurs in all situations.
Situational — Occurs only under specific circumstances.

What are the conditions under which orgasm can occur with normal functioning?
・ There are differences in the intensity, type, and duration of stimulation required for women to reach orgasm. Most women need some level of clitoral stimulation to experience orgasm.

・ Indirect clitoral stimulation, such as through vaginal intercourse, may be sufficient for most women to trigger orgasm, while other women may respond more quickly and reliably to direct clitoral stimulation.

・ Anatomical differences between women may explain why some women achieve orgasm more easily through vaginal intercourse alone.

・ The inability to reach orgasm through vaginal intercourse is not a hindrance, but a typical (sometimes adjustable) variation in sexual response.

What factors might cause difficulty achieving orgasm?
Female orgasmic difficulties are multifactorial and may be related to inhibitory psychosocial factors involving the individual, their partner, and/or sociocultural background.

Relationship Factors:
・ Concerns or negative thoughts about one's sexual function, and the ability to be distracted or interrupted during sexual excitement.
・ Those who experience fewer sexual fantasies tend to have a lower frequency of sexual orgasm.

Relational factors:
・ Difficulty communicating about sexual topics with a partner.
・ Lower relationship satisfaction is associated with a lower frequency of orgasms during sexual intercourse and a longer time to reach orgasms.
・ A partner’s sexual function and willingness to engage in foreplay can influence a woman’s likelihood of reaching orgasms.

Sociocultural Factors:
・ Barriers that make it more difficult for women to express sexual comfort and experience orgasm.
・ Feelings of sexual guilt are associated with orgasmic difficulties.

Medical and Medication-Related Causes:
・ Antidepressants
・ Antipsychotic medications, especially first-generation (typical) agents
・ Pelvic surgery or vaginal childbirth, which may lead to a temporary decrease in orgasmic function
・ Pelvic Radiation Therapy

Cognitive Behavioral Therapy
Cognitive-behavioral therapy is a multi-component intervention that selectively uses behavior and cognitive techniques based on the specific needs of each partner. Multiple components can be effectively combined to treat various factors that cause female orgasm disorders, including:

MASTURBATION TRAINING
・ Teaches women to reach orgasm through self-stimulation and eventually reach orgasm in partnered sex.
・ Helps women become familiar with their bodies and genitals, identify pleasurable areas, and directly stimulate those areas.
・ May involve the use of erotic videos, images, or text, and/or vibrators as aids.
・ Once a woman can reliably achieve orgasm, her partner is gradually involved in the exercises so they can learn how to facilitate orgasm during sexual activity.

Feeling focused exercises
・ Partner-directed treatment aims to increase partner comfort for sexual intimacy and reduce avoidance of fear experiences.
・ Focuses on shifting the goal of sexual activity toward sensory pleasure rather than solely achieving orgasm.

Exposure Therapy
・ Used to treat avoidance of sexual intercourse caused by anxiety in female orgasmic disorder, helping increase comfort with feared experiences and reduce avoidance behaviors.
・ Targets specific situations (e.g., being seen naked) to reduce anxiety and avoidance.

Mindfulness Training
・ May enhance awareness of sexual arousal and pleasure, while reducing distractions that inhibit arousal and orgasm.

Treatment and Management Options
Stimulation Device :
Clitoral vacuum attractors and vibrators are sometimes recommended to enhance genital engorgement and sexual arousal.

Medications :
Currently, no medications or dietary supplements have demonstrated sufficient evidence of effectiveness to support routine use in treating this condition. However, certain hormonal therapies may be used as adjunctive treatment in specific cases.

In 2010, the Journal of Sexual Medicine noted that body image plays a significant role in self-confidence and sexual satisfaction. Therefore, improving confidence in the appearance of one’s intimate area may also help increase the likelihood of achieving orgasm.
